Ozarks Community Hospital in Springfield, MO has an immediate opening for a full-time Patient Navigator within our Medical Home Department.

Specialty clinic services include Pediatrics, Women’s Health, Diabetes Care, Hepatitis C, Urology, Geriatrics, Pain Management, and the FLAG (Fatty Liver and Goals) Clinic.

Job Summary: The Patient Navigator is responsible for guiding patients through the healthcare system, offering support and resources to ensure they receive appropriate care in a timely and efficient manner. The Patient Navigator will serve as a critical point of contact for patients, helping them navigate through healthcare services, community resources and coordination of their care. This role involves assessing patients’ needs, addressing potential barriers to care, and providing education and emotional support. The Patient Navigator works closely with clinical and administrative teams to streamline patient experiences and enhance access to quality healthcare.
Requirements:
- High school diploma or equivalent.
- Certification as a Patient Navigator is preferred but not required.
- Prior experience in healthcare, social services, or patient advocacy role is preferred.
- Familiarity with healthcare systems, community resources, and medical terminology.
- Prefer Medical Assistant
