Ozarks Community Hospital in Springfield, MO has an immediate opening for a full-time Community Health Worker within our Medical Home Department.

Specialty clinic services include Pediatrics, Women’s Health, Diabetes Care, Hepatitis C, Urology, Geriatrics, Pain Management, and the FLAG (Fatty Liver and Goals) Clinic.

Job Summary: The CHW will be an integral part of the client resource team. CHW will help to bridge patients to resources to reduce social determinants of health that impact clients’ health and well-being. Provides informal coaching, counseling and social support, culturally appropriate health education, and basic screening services. Ability to perform duties consistent with Standards of Practice, goals and philosophy of Ozarks Community Hospital and the Employee Code of Conduct.

Requirements:

  • High school diploma or equivalent.
  • Must obtain CHW certification through an approved curriculum within 12 months of hire date.
  • Valid Driver’s License required.  This position may require a Motor Vehicle Driving Record Background Check to be completed during the onboarding new hire process and throughout employment.
  • Must have a valid Class E license upon hire or Class E license must be obtained during orientation period.
  • Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class. Throughout employment, current BLS certification must be maintained by renewal during renewal period or at next available scheduled class.
  • Bilingual Preferred
