Ozarks Community Hospital in Rogersville, MO has an immediate opening for two full-time Long Term Care License Practical Nurses (LTC LPN) within our Medical Home Department.

Job Summary: Provides direct and indirect patient care in partnering regional long term care facilities.  Provides care that meets the psychosocial, physical, and general aspects of care. Meets the communication needs of patient and family. Provides care that reflects initiative and responsibility indicative of professional expectations, under the scope of licensure.  Maintains regulatory requirements, nursing and facility policies, procedures, and standards.  Communicates with physicians and team members about patient's clinical condition, including results of diagnostic studies and symptomatology.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current Licensed Practical Nurse licensure in the state of practice.
  • Minimum of one (1) year experience in the outpatient setting is preferred.
  • Current BLS certification upon hire or BLS certification obtained during orientation period. Must maintain current BLS certification throughout employment.
